It is my understanding that notwithstanding the exercise of due care and control of the attending dogs whether on my part, those of the other owner/handlers and employees of A1tailwaggers Incorporated, these obedience classes are not without risk, not only to myself, but also to family or guests who may attend as well as to my dog.
I hereby agree to indemnify and hold harmless A1tailwaggers Incorporated, its owners, trainers, employees, representatives and landlords from any and all liability for any loss, damage or injury to any person, any animal or anything, that may have arisen out of, or is in any way connected with, participation in training classes held by A1tailwaggers Incorporated., at their various training locations. Furthermore, I assume all responsibility for any loss, damage or injury to myself, my guests, my dog or my property, which may have been caused by negligence, or any action or inaction, of any person connected in any way with the above said classes.  
I hereby certify that my dog(s) that I enter into these classes,by the time of the first session, will have received up to date vaccinations. For puppies this will include Canine Distemper, Parvovirus and Canine Viral Hepatitis
Dogs 6 month and older Rabies.
Have their veterinarian's permission to attend.
